The Commonwealth Government has developed an annual Health Assessment for people aged 75 years and over.
The government is encouraging General Practitioners to take an in-depth assessment of your health. Queens Road Surgery is able to provide a detailed, comprehensive Health Review for you.
Aim
To identify potential areas of your health and lifestyle in which assistance can be arranged to aid in keeping you healthy and safe.
Who
The health review is completely voluntary and is available once a year to all people 75 years and over. If you have already had a health assessment through another practice or an ACAT assessment, please let us know as you are only eligible for 1 health assessment per year.
When
A mutually convenient time will be arranged for you to attend Queens Road Surgery and visit one of our friendly nurses for approximately 1 hour. Please bring along all your medications. A follow up visit will be arranged with your doctor at a later date to review the nurse assessment.
Cost
The cost of the Health Assessment is completely covered by Medicare
